Students seeking a leave of absence must complete the leave of absence form, which indicates the reasons for this request and the length of the absence.  This form must be submitted to the major director for approval.  Students requesting the leave of absence who plan to continue enrollment in school must meet with the major director to develop an appropriate academic plan for the interim time period.  This plan must be submitted with the leave of absence form (dlvabs.pdf).

Students who leave for more than one year must reapply to their program for re-admittance.
